A HIPAA consent to release information

Most settings readily release information about patient’s life-threatening illnesses to their healthcare representatives but often refuse to do so when the requests come from others. If you would like others to have access to your records it helps if you execute a HIPAA release of information for them. The form provided here is generic and should be generally acceptable. But it can be useful if you ask Institution that has your records has its own form, in which case that is the one that you should execute.
